How To Apply

Thank you for your interest in the UAMS Addiction Medicine Fellowship Program. To be considered for our program, you must have completed a residency in one of the following:  Anesthesiology, Emergency Medicine, Family Medicine, Internal Medicine, Obstetrics and Gynecology, Pediatrics, Preventive Medicine, or Psychiatry.

Applications are being accepted for the academic year 2022-2023. The following information should be submitted to Berva Bentley, 1210 Wolfe Street, Slot 654, Little Rock, AR 72202:

  1. A current CV
  2. A letter of recommendation from your training director including your date of completion (or anticipated date) and standing in the program; this letter is to be sent directly by training director.
  3. Two additional letters of recommendation; these letters are to be sent directly by the authors of the letters.
  4. An original copy of your medical school transcript; Sent directly from the medical school.
  5. A copy of your USMLE or COMLEX transcript (with all three scores)
  6. ECFMG certificate (for foreign medical school graduates)
  7. Medical School Transcript
  8. Medical School Performance Evaluation (MSPE)
  9. A personal statement regarding your interest in the field of addiction medicine

Upon receiving the above information, the application packet will be reviewed by the fellowship director.

For the academic year 2023-2024, we will accept applications through the Electronic Residency Application Service (ERAS).  Match registration opens on August 24, 2022.  We will make a selection through the National Resident Matching Program (NRMP) Medicine and Pediatric Specialties Match (formerly known as the Medical Specialties Matching Program) on November 30, 2022.
